To book an appointment:
Select 'Book Appointment' follow the instructions to complete the booking process.
If you need assistance rescheduling, call the clinic administration office (405)822-3802. However, you should be able to login to the portal, choose 'My Appointments', and make any scheduling changes.
Check your EMAIL after you book an appointment. The intake package will be sent via email or available in the patient portal under 'My Forms'. Intake forms must be complete prior attending the telehealth/virtual appointment with a physician.
In-person appointments are not required to complete the forms prior to arriving at the clinic.
Payment is due in full prior to speaking with the physician for a medical marijuana recommendation consult.
State and processing fees are not included in a medical marijuana consultation fee and are assessed after the intake process is complete.
To view or pay a complete billing summary, select 'My Invoices'. An updated billing invoice will be accessible in the portal once the intake documents have been completed. A finalized invoice will be provided after the conclusion of the provider visit.
Alert the staff if your child will miss school due to their scheduled appointment. You may request an excused absence letter for school/work from the physician during consultation.
