To schedule an appointment, select 'Book Appointment' and follow the provided steps.
For rescheduling, contact the clinic administration office at (405)822-3802 or log in to the portal, select 'My Appointments,' and make the necessary changes.
After booking, check your EMAIL for the intake package, which will also be available in the patient portal under 'My Forms.' Intake forms must be completed before a telehealth or virtual appointment with a physician.
For in-person appointments, completing the forms before arriving at the clinic is not required.
Full payment is required before consulting with the physician for a medical marijuana recommendation. Consultation fees exclude state and processing fees, which are assessed after the intake process. To view or pay your billing summary, select 'My Invoices.' An updated invoice will be available in the portal after completing the intake documents, with a finalized invoice provided after the provider visit.
If your child will miss school due to their appointment, please inform the staff. You may request an excused absence letter for school or work from the physician during the consultation.
