New clients who book their initial session online are required to provide the following information in the Notes section on the booking page:
1. First & Last Name of Client(s)
2. Date of Birth (Month dd, yyyy)
3. A brief summary of the reason(s) for scheduling the appointment.
A TPS staff member will also contact you via email or phone and provide forms to complete before your appointment.
