Current meds/supplements + allergies/sensitivities (latex/adhesive included)
Relevant conditions (e.g., heart/kidney disease, pregnancy/breastfeeding)
Emergency contact
Your information is collected under BC’s PIPA and stored securely in our EMR (Zanda). We don’t share it without your consent except where required by law.

Booking a group session?
Please provide the names and contact information of the attendees, and share the client booking portal link with them. This will enable them to sign up and link to the group booking.
